Periodontal Maintenance in the Financial District (94104)
Ongoing Gum Disease Management • Downtown San Francisco
Periodontal maintenance (CDT D4910) is a specialized, clinical-grade cleaning designed specifically for patients who have already undergone treatment for active periodontal (gum) disease—such as scaling and root planing (deep cleaning) and/or surgical intervention. This vital preventive service is typically scheduled every 3 to 4 months to suppress pathogenic bacteria and prevent the recurrence of chronic periodontitis (ICD-10: K05.3).
What is Included in a Periodontal Maintenance Visit?
- Thorough Debridement: Professional cleaning both above and below the gum line.
- Targeted Decontamination: Precise removal of stubborn plaque, hardened calculus (tartar), and bacterial biofilms from deep gum pockets.
- Periodontal Charting: Continuous monitoring, measurement, and clinical charting of pocket depths and gum attachment levels.
- Localized Therapeutics: Application of targeted antimicrobial agents or localized antibiotics when clinically indicated.
- Laser Therapy Support: Utilization of advanced soft-tissue laser treatment locally as needed to eliminate residual pathogens.
Ongoing Gum Disease Management Across Downtown San Francisco
Unlike a routine dental prophylaxis, periodontal maintenance is an essential component of an ongoing, long-term disease management strategy. It protects your teeth, soft tissues, and underlying alveolar bone structure from progressive deterioration over time.
Why Periodontal Maintenance Is Essential After Deep Cleaning
Initial scaling and root planing (deep cleaning) successfully eradicates active bacterial infections and smooths root surfaces beneath the gum line. However, because periodontal disease is a chronic condition, harmful bacteria naturally begin to repopulate periodontal pockets within 90 to 120 days.
Ongoing periodontal maintenance is non-negotiable because it regularly disrupts these pathogenic colonies before they can reattach, permanently halting bone loss, preventing recurrent infections, and safeguarding your natural smile against premature tooth loosening.
Keep your smile healthy and your gum disease under control.

Medical Condition
Gingival Embrasures (Black Triangles)
Interdental papilla recession—often seen as small gaps or 'black triangles' between your teeth—can disrupt the natural contours of your gum line. These spaces often develop due to bone volume changes or natural tissue maturation, and they can unfortunately become areas where debris accumulates, making oral hygiene more difficult. We provide a precise, non-surgical restorative treatment designed to reconstruct the gum papilla. This restores the natural architecture of your smile while improving your ability to keep those areas clean and healthy, effectively strengthening the overall integrity of your gums.
Medical Procedure
Clinical Periodontal Maintenance Therapy
A comprehensive clinical protocol performed at 100 Montgomery St, designed to stabilize periodontal health and restore gingival aesthetics. Dr. Tabar combines precise subgingival instrumentation to eliminate pathogenic biofilm within periodontal pockets with the advanced Bioclear method. This dual-focus approach not only arrests inflammatory progression but also restores the natural height of the interdental papilla, effectively closing embrasure voids to support both a healthy, cleansable gum line and a harmonious smile.
Medical Device
AI Cavity and Bone Loss Detection Software
X-rays used at Digital Dental Practice SF to analyze and flag subgingival decay and early-stage bone loss for proactive periodontal care.

Clinical FAQs
Why is periodontal maintenance needed every 3-4 months?
Clinical studies show that harmful periodontal bacteria repopulate deep pockets within 90 days. Most patients at Digital Dental Practice require maintenance every 3 months to stop the inflammatory cycle that leads to bone loss.
Can Bioclear fix the gaps (black triangles) between my teeth?
Yes. Black triangles, or gingival embrasures, are often caused by bone loss or recession. Dr. Tabar uses the Bioclear Method at our Financial District office to non-surgically close these gaps using high-end composite resin.
How does AI technology help prevent gum disease?
At Digital Dental Practice, we use AI to analyze X-rays with clinical precision, identifying early bone density changes and subgingival issues that might be missed by the human eye alone.

Dr.Tabar is the best dentist I have had in my life - and that’s saying a lot since I have moved a lot and had many dentists. I trust her advice completely and know that she always puts my needs first and gives me an honest assessment of what is happening. Under her careful care and guidance, we were able to improve my gum disease even through a pregnancy and breastfeeding. She was also quick to identify and resolve dental issues I had while taking care of a newborn, managing the issue quickly and at lowest cost. I ask a million questions and at every interaction she is always deeply patient and communicative. I am a bit bummed to leave her since we are moving. But I would highly recommend her practice to anyone looking for an excellent dentist who always does the right thing and is highly knowledgeable, personable, and communicative.Posted on Patient Connect365![]()

I literally have a phobia of dentists and was very nervous when I moved to SF and left my old dentist behind. I cannot believe my luck that I found Dr Tabar, she is amazing! I still have the phobia but I can manage to go and see her four times a year (for 4 years) and totally trust her. She took the time to understand how I feel and was genuine in her interaction with me. My teeth are 100% better since I have been going to her and she has managed to get me on a very strict hygiene ritual - nobody has managed that before! I don't feel she does things for money but rather what is best for you. Her office is lovely and her staff great. I cannot recommend her highly enough.Posted on Patient Connect365![]()

I loved my childhood dentist. I went to him for 15+ years and within that time he managed to stay up-to-date with all the dental technology. I was sad to leave him and his wonderful practice when I moved to San Francisco. My first new dentist in SF was William R Smith (http://www.yelp.com/biz/william-r-smith-dds-cow-hollow-dental-center-san-francisco). He works with his daughter, Dr. Connelly. Both were very pleasant people, but I'm sad to say their practice is completely out-dated, which I found surprising given his daughter RECENTLY GRADUATED from UoP. I walked away from my bi-annual dental exam with a quick brush and floss, 20 minutes max-no gum measuring, fluoride treatment, anything else. Why else do we have dental insurance? I was worried that maybe all other dentists were actually like this, and it was only my childhood dentist who went above and beyond. I decided to give Dr. Tabar a chance because something just did not feel right about leaving my dental care in the hands of Dr. Smith and I COULD NOT BE HAPPIER! I filled out all of my "paperwork" completely on the computer with the guidance of her wonderful receptionist. I then took x-rays with her dental hygienist, who was just as sweet, all while watching Netflix on a screen in front of me. Dr. Tabar's exam was as thorough/gentle/amazing as it could be. Her technology even allowed her to take pictures and pull them up on the screen in front of me. She took the time to explain everything in detail, even a minor scrape on the roof of my mouth. Her wonderful collaboration with her hygienist made me feel confident I was putting my dental care in the right hands. In addition, Dr. Tabar's office almost feels like a spa with the updated decor, air-massage chairs and scented spa-towelette they give you after your visit to wipe off your face. NOT ONE THING will disappoint you about choosing Dr. Tabar as your dentist. I went there expecting only detailed cleaning, but was seriously blown-away by the exemplary service I received. She should run a side-business on teaching other dentists how to run their offices.Posted on Patient Connect365![]()

Just finished a visit with a fantastic new dentist in downtown San Fra ncisco - Dr. Maryam Tabar. First of all let me say it's really nice that someone finally gets it - for those of us who have to work during the week she's open at no extra charge for appointments on Saturday. She is the only dentist working in the office and along with her assistant is doing all of the work - from the cleaning to the fillings. She says that because of this she has a chance to know the conditions and needs of her patients teeth. Second the office itself is brand new with all modern equipment: computerized records and forms, new special drills and cleaning stuff, the chairs even give you a massage. Overall the office has a modern professional feeling and is decorated in a soothing colors that are subtly calming. I put going to the dentist on the list of fun things to do just under ... well, let's just say I don't like it. With they're sharp metallic instruments and screaming drills it usually feels like you've signed up to be the demonstration dummy for a class on S&M. In contrast, Dr. Tabar was very gently. She used a sonic scraper (I think that's what it's called) instead of the metal scraping and it was sooo much better. She is also extremely through - carefully checking for everything from cavities and gum disease to oral cancer. I'm never going to like going to the dentist but I have at last found one I feel comfortable seeing.
